List Display and Data Entry screen.
As I'm doing a lot of editing in the titles, dates and constituants, it happens frequently in the above-mentioned interactions. Less frequently betwee List display and Data Entry.

What fields are you working in when this happens? This has been an ongoing problem for us and it seems to be related to adding thesaurus terms through attributes.

This issue has been discussed recently - apparently the reason mostly seems to be the use of an incorrect ODBC driver. See the message of Danielle on February 10, 2014:

All PC-s using TMS 2012 need to have SQL Server Native Client 10.0 or 11.0 installed (and not SQL Server!). If you don't see this option in the list of available drivers, it means you first need to find (download) the correct file (sqlncli10.dll or sqlncli11.dll) and place it into the Windows system directory.
