We track our incidents in TMS, in the conservation section.
Activity type: Incident report.
We populate examiner 1 and 2 based on who responded to the incident and who wrote the report.
We use attributes for controlled vocabulary for type of incident, status and location.
We use the overall analysis field for a written description of what happened.
We link any documents as a line item under the activity.

Dear colleagues
I’ve been discussing starting to log reports of damage to paintings in TMS with my curatorial colleagues. I see there is no ‘loss / damage’ module in TMS,* so am considering using the Conservation module. But before I
do anything, I’d be interested to know how the rest of you are recoding this kind of information – and, if you’re willing to share them, I’d love to have a look at your guidelines for doing so.
